Using the Quote System

Use Internet Explorer to obtain quotes (most other browsers will not work). Some carriers will provide quotes only if you use a PC and not a Mac. Quotes obtained from this system are not as accurate (and therefore should be considered an approximation) as those obtained from an agent who can quote on each individual carrier’s quoting system and make the correct adjustments for build, smoking status, prior insurance coverage, and medical history.

Step 1:  Basic Information

  • Enter your ZIP code.  All other information in this step is voluntary.
  • Click “Next Step” to proceed.


Step 2:  Personal Profile

  • Enter information for all items (whether or not asterisked) including gender, birth date, height, weight and smoking status.  Enter Yes for "Student" for all  children whether or not they are in school (some carriers will not include children in the quote unless a child is indicated as a student).  This will make the quotes as accurate as possible.  Note that all carriers except CoventryOne HMO (formerly Vista) adjust rates for smoking status. If you indicate that you are a smoker, rates for all carriers except Aetna and AvMed will be changed to reflect smoking status; Aetna and AvMed would notify you of the adjusted rate when they process your application.   Aetna and AvMed do not adjust for smoking status for individuals with low BMI scores with no health conditions Also, all carriers except CoventryOne (both the HMO and PPO plans have only one build table for all eligible applicants) adjust rates for build if you do not meet their preferred build tables; however, only the UnitedHealthOne quotes are adjusted on this system for build—and they are adjusted only for their Preferred and Standard 1 build tables; if you fall into their Standard 2 build table you will be quoted as Standard 1 on this system.  All carriers will quote irrespective of your weight and whether or not you qualify based on build; if you exceed the carrier’s maximum build parameters they will not offer you coverage and will notify you accordingly.
  • UnitedHealthOne’s quotes for Preferred assume that you have had coverage within 63 days of the effective date and are adjusted to reflect their prior coverage discount; however, you will not be eligible for this discount if you have not had coverage within this period and rates will be increased 7% to reflect ineligibility for the discount.  UnitedHealthOne also includes a discount card that is an additional $4 and not included in the quotes.
  • Humana quotes at the Preferred rate if you are a non-smoker (there is no Preferred Smoker rate) or at their Standard Smoker rate if you are a smoker, irrespective of whether or not you qualify for these rates.  [Humana has three different rate classes (Preferred, Standard and Sub-standard) and, the two non-Preferred classes have both Smoker and non-Smoker rates.]    If you don’t qualify for the quoted rates, Humana’s application system adjusts the rate when you are completing the application process.  Please call 877-734-3884 to determine if you qualify for the quoted rate.
  • Click Add Spouse and/or Add Child as appropriate and enter all requested personal information.
  • Select a date for “Desired Coverage Start Date.”  The system defaults to the next succeeding 1st or 15th of the month (CoventryOne HMO and PPO plans enroll only as of the first of the month, so always choose a first-of-the-month date for these carriers).
  • Click on “View Quotes” to proceed.


Step 3:  View Plans

  • All available plans included in the quoting system will be listed in cost order (lowest to highest cost).
  • Quotes are available for over 190 different plans, depending on your zip code. Reduce the number by using the “Limit Plans By” feature.  Customize your search by entering your choices in these boxes:
    • Plan Type
    • Deductible
    • Coinsurance
    • Copay
    • Carrier (Note: UnitedHealthOne is listed as "Golden Rule " on the system.)
  • CoventryOne HMO and CoventryOne PPO are both included together in one (“CoventryOne Florida”) listing.  Click “HMO” under “Plan Type” to obtain HMO quotes and “PPO” under “Plan Type” to obtain PPO quotes.  Click “All” under “Plan Type” to obtain both HMO and PPO quotes.  Note:  the HMO can be purchased in Miami-Dade, Broward, Palm Beach, Martin and St. Lucie Counties but the PPO is available only in the first three counties in South Florida.  The HMO is not available in North Florida, but the PPO can be purchased  in Hillsborough, Pinellas, Pasco, and Hernando counties.
  • For AvMed, Humana and UnitedHealthOne, you can click on “View Optional Benefits” and add available optional benefits to the quote.  Click “Add to Plan,” and the price will change. Aetna plans are quoted without dental but all plans can be purchased with dental.  Although you can add a dental option to your Humana quote, Humana also has two stand-alone dental plans that can only be quoted from a different system.  Call us at 877-734-3884  to ask about other available options and for quotes from carriers not included in this quoting system (including the two Humana stand-alone dental plans and the Aetna dental plan).
  • You can compare between 2 to 4 plans by using the “Compare” feature.  Select plans by checking the box at the extreme left and then click “Compare.”
    • Click “Email Quotes” or “Print Quotes” buttons at the top of the quote (above the plan listings) if desired.
    • Click “Provider Lookup” to locate providers in the network included in the quote.
    • Click “see brochure” to download a brochure.
    • Click “More Details to determine” if more information is available about the plan.


Step 4: Apply Today


Click on the “Apply” button to make application.  We suggest that you contact us at 877-734-3884 for help in completing your application on the system.

Carriers will  adjust rates and many carriers will exclude (“rider”) for certain medical conditions and medications.

Quotes are provided whether or not you qualify for coverage, and carriers will accept applications even if you do not meet their requirements. You will be rejected for coverage if you do not qualify. Before applying call us at 877-734-3884 to determine qualification requirements.

Coventry One HMO is the only carrier that does not adjust premiums for smoking status, and CoventryOne HMO and PPO are the only carriers that have only one rate for all weight categories.

Automated quotes should be used only as an indicator of general differences between plans and carriers and as a rough gauge of premiums.
